But these donor zones behave nothing like the back of the head, and the FUE punch that performs beautifully on the scalp can struggle on the chin, chest or trunk. Beard hair: thick shafts, mobile skin
Beard follicles are typically coarse, with a thick shaft and a deep, robust root. The skin of the face and neck is also more mobile and more elastic than scalp skin, which means it deforms under the punch instead of letting the edge bite cleanly.
Two adjustments follow from this:
- A slightly larger diameter is often appropriate to accommodate the thick shaft and avoid clipping the follicle. Many surgeons use 0.9 mm to 1.0 mm in the beard.
- Counter-traction is essential. Because the skin is so mobile, an assistant or the surgeon must stretch the area taut so the punch scores rather than tents the skin.
Beard roots also tend to sit deeper, so depth control and a punch that holds its edge through repeated coring both matter. A dull edge on mobile skin is a recipe for transection and bruising.
Body hair: variable depth and angle
Chest, abdomen, back and limb hair vary enormously in caliber, density and angle, sometimes within the same patient. Body hair often grows at a shallow, acute angle to the skin, which makes the entry geometry harder than on the scalp.
For body donor zones, surgeons commonly favour:
- Smaller diameters, often 0.8 mm to 0.9 mm, because individual body follicles are frequently single-hair units.
- Sharp, low-friction edges that can engage skin at an acute angle without skidding.
- Shallow, careful scoring because body follicles can be surprisingly short and easy to amputate.
Hyperhidrosis, oil and grip
Faces and torsos produce oil and sweat that the scalp does not, and that film reduces the grip between punch and skin. A serrated FUE punch can help here by scoring the surface more positively, while a very sharp flat punch reduces the force needed for entry. Many BHT surgeons keep both edge types available and choose per zone.
Wound healing in cosmetically visible areas
The donor management conversation changes when you harvest from the face or chest. Scarring on the beard or torso is more visible to the patient than scarring hidden in scalp hair, so minimising wound size is a priority. This pushes surgeons toward the smallest diameter that will not transect, and toward consistent, sharp punches that produce a clean, small wound every time.
Why edge consistency is non-negotiable in BHT
Body and beard cases are long, repetitive and unforgiving. A punch whose edge degrades over a few hundred extractions will start tearing tissue exactly when fatigue is also setting in for the surgeon.
